ref-manual: Added descriptions for four new variables

Fixes [YOCTO #7665]

Added descriptions in the glossary for the following variables:

 * SDK_HOST_MANIFEST
 * SDK_TARGET_MANIFEST
 * SDK_VERSION
 * TOOLCHAIN_OUTPUTNAME
