documentation: Manual reference fixes for mega-manual processing
I fixed several occurences of cross-references to whole manuals where the formatting was split in the <ulink> statement. The <ulink> tag and the manual title must be on one DocBook line in order for the sed scripts to correctly process the links when the mega-manual is created.
