core: Cleanup gerrit and JIRA references. (#58)

* Remove .gitreview and switch to pulling the main asterisk branch
  version from configure.ac instead.

* Replace references to JIRA with GitHub.

* Other minor cleanup found along the way.

Resolves: #39
