Stasis performance improvements
This patch addresses several performance problems that were found in
the initial performance testing of Asterisk 12.
The Stasis dispatch object was allocated as an AO2 object, even though
it has a very confined lifecycle. This was replaced with a straight
ast_malloc().
The Stasis message router was spending an inordinate amount of time
searching hash tables. In this case, most of our routers had 6 or
fewer routes in them to begin with. This was replaced with an array
that's searched linearly for the route.
We more heavily rely on AO2 objects in Asterisk 12, and the memset()
in ao2_ref() actually became noticeable on the profile. This was
#ifdef'ed to only run when AO2_DEBUG was enabled.
After being misled by an erroneous comment in taskprocessor.c during
profiling, the wrong comment was removed.

Taskprocessor optimization; switch Stasis to use taskprocessors
This patch optimizes taskprocessor to use a semaphore for signaling,
which the OS can do a better job at managing contention and waiting
that we can with a mutex and condition.
The taskprocessor execution was also slightly optimized to reduce the
number of locks taken.
The only observable difference in the taskprocessor implementation is
that when the final reference to the taskprocessor goes away, it will
execute all tasks to completion instead of discarding the unexecuted
tasks.
For systems where unnamed semaphores are not supported, a really
simple semaphore implementation is provided. (Which gives identical
performance as the original taskprocessor implementation).
The way we ended up implementing Stasis caused the threadpool to be a
burden instead of a boost to performance. This was switched to just
use taskprocessors directly for subscriptions.

Optimize how Stasis forwards are dispatched
This patch optimizes how forwards are dispatched in Stasis.
Originally, forwards were dispatched as subscriptions that are invoked
on the publishing thread. This did not account for the vast number of
forwards we would end up having in the system, and the amount of work it
would take to walk though the forward subscriptions.
This patch modifies Stasis so that rather than walking the tree of
forwards on every dispatch, when forwards and subscriptions are changed,
the subscriber list for every topic in the tree is changed.
This has a couple of benefits. First, this reduces the workload of
dispatching messages. It also reduces contention when dispatching to
different topics that happen to forward to the same aggregation topic
(as happens with all of the channel, bridge and endpoint topics).
Since forwards are no longer subscriptions, the bulk of this patch is
simply changing stasis_subscription objects to stasis_forward objects
(which, admittedly, I should have done in the first place.)
Since this required me to yet again put in a growing array, I finally
abstracted that out into a set of ast_vector macros in
asterisk/vector.h.

Remove dispatch object allocation from Stasis publishing
While looking for areas for performance improvement, I realized that an
unused feature in Stasis was negatively impacting performance.
When a message is sent to a subscriber, a dispatch object is allocated
for the dispatch, containing the topic the message was published to, the
subscriber the message is being sent to, and the message itself.
The topic is actually unused by any subscriber in Asterisk today. And
the subscriber is associated with the taskprocessor the message is being
dispatched to.
First, this patch removes the unused topic parameter from Stasis
subscription callbacks.
Second, this patch introduces the concept of taskprocessor local data,
data that may be set on a taskprocessor and provided along with the data
pointer when a task is pushed using the ast_taskprocessor_push_local()
call. This allows the task to have both data specific to that
taskprocessor, in addition to data specific to that invocation.
With those two changes, the dispatch object can be removed completely,
and the message is simply refcounted and sent directly to the
taskprocessor.

When originating channels, ast_pbx_outgoing_* caused the dialed channel
reference to be bumped twice. Ostensibly, this routine is bumping the channel
lifetime such that the channel doesn't get nuked in between locks/unlocks;
however, since the routine should return the dialed channel with its
reference bumped, it only needs to do this one time.

This adds a new dialplan application, SayAlphaCase, that performs much
the same function as SayAlpha except that it takes additional options
which allow the user to specify whether the case of each letter should
be announced for uppercase, lowercase, or all letters. Similar
functionality has been added to the SAY ALPHA AGI command via an
optional parameter.

This removes unused code, event types, IE pltypes, and event IE types
where possible and makes several functions private that were once
public. This includes a renumbering of the remaining event and IE types
which breaks binary compatibility with previous versions. The last
remaining consumers of the old event system (or parts thereof) are
main/security_events.c, res/res_security_log.c, tests/test_cel.c,
tests/test_event.c, main/cel.c, and the CEL backends.

When asterisk has run out of memory (for whatever reason), the alloc
function logs a message. Logging requires memory. A recipe for
infinite recursion.
Stop the recursion by comparing the function call depth for sane values
before attempting another OOM log message.

If the thread servicing the dial request isn't created successfully, the
outgoing dial lock will still be held when the function returns. This patch
unlocks the lock on this off nominal path.

This makes it so that we can detect failures to originate as with
earlier versions of Asterisk, which restores the Asterisk 11 behavior
for the originate manager action. This was causing the ACL tests for
SIP and IAX2 to fail since those tests expected originate failures
when ACLs would cause rejections. Also, this patch fixes crashes in
chan_sip when ACLs rejected peers during registration verification.

The new res_ari_asterisk.so module presents several config options
from asterisk main. Unfortunately, they aren't exported, so the module
won't load on Linux.
This patch renames the variables, adding the ast_ prefix so they will
be exported.

In working with res_stasis, I discovered a significant limitation to
the current structure of stasis_caching_topics: you cannot subscribe
to cache updates for a single channel/bridge/endpoint/etc.
To address this, this patch splits the cache away from the
stasis_caching_topic, making it a first class object. The stasis_cache
object is shared amongst individual stasis_caching_topics that are
created per channel/endpoint/etc. These are still forwarded to global
whatever_all_cached topics, so their use from most of the code does
not change.
In making these changes, I noticed that we frequently used a similar
pattern for bridges, endpoints and channels:

This pattern was extracted as the 'Stasis Caching Pattern', defined in
stasis_caching_pattern.h. This avoids a lot of duplicate code between
the different domain objects.
Since the cache is now disassociated from its upstream caching topics,
this also necessitated a change to how the 'guaranteed' flag worked
for retrieving from a cache. The code for handling the caching
guarantee was extracted into a 'stasis_topic_wait' function, which
works for any stasis_topic.

This adds CEL support for blind and attended transfers and call pickup.
During the course of adding this functionality I noticed that
CONF_ENTER, CONF_EXIT, and BRIDGE_TO_CONF events are particularly
useless without a bridge identifier, so I added that as well.
This adds tests for blind transfers, several types of attended
transfers, and call pickup.
The extra field in CEL records now consists of a JSON blob whose fields
are defined on a per-event basis.

This patch does the following:
* It adds a new soft hangup flag AST_SOFTHANGUP_HANGUP_EXEC that is set when a
channel is executing dialplan hangup logic, i.e., the 'h' extension or a
hangup handler. Stasis messages now also convey the soft hangup flag so
consumers of the messages can know when a channel is executing said
hangup logic.
* It adds a new channel flag, AST_FLAG_DEAD, which is set when a channel is
well and truly dead. Not just a zombie, but dead, Jim. Manager, CEL, CDRs,
and other consumers of Stasis have been updated to look for this flag to
know when the channel should by lying six feet under.
* The CDR engine has been updated to better handle a channel entering and
leaving a bridge. Previously, a new CDR was automatically created when a
channel left a bridge and put into the 'Pending' state; however, this
way of handling CDRs made it difficult for the 'endbeforehexten' logic to
work correctly - there was always a new CDR waiting in the hangup logic
and, even if 'ended', wouldn't be the CDR people wanted to inspect in the
hangup routine. This patch completely removes the Pending state and instead
defers creation of the new CDR until it gets a new message that requires
a new CDR.

The originate APIs allow callers to provide a pointer to a channel that will
point to the originated channel if the function call succeeds. This is used by AMI
to provide channel information when the originate is performed synchronously.
Unfortunately, if the originate fails in certain ways, the outbound channel is
already disposed of during the dialing itself. This results in the channel being
improperly dereferenced by the internal originate function in pbx.c.
This patch ref bumps the channel to prevent this from occurring. Callers must now
unlock and unref the channel (which is more in line with general channel management
guidelines anyway).
This only affects manager, as it is the only consumer of this API function that
actually passes in a channel pointer.

This patch is the initial push to update Asterisk's CDR engine for the new
bridging framework. This patch guts the existing CDR engine and builds the new
on top of messages coming across Stasis. As changes in channel state and bridge
state are detected, CDRs are built and dispatched accordingly. This
fundamentally changes CDRs in a few ways.
(1) CDRs are now *very* reflective of the actual state of channels and bridges.
This means CDRs track well with what an actual channel is doing - which
is useful in transfer scenarios (which were previously difficult to pin
down). It does, however, mean that CDRs cannot be 'fooled'. Previous
behavior in Asterisk allowed for CDR applications, channels, and other
properties to be spoofed in parts of the code - this no longer works.
(2) CDRs have defined behavior in multi-party scenarios. This behavior will not
be what everyone wants, but it is a defined behavior and as such, it is
predictable.
(3) The CDR manipulation functions and applications have been overhauled. Major
changes have been made to ResetCDR and ForkCDR in particular. Many of the
options for these two applications no longer made any sense with the new
framework and the (slightly) more immutable nature of CDRs.

The attended transfer API call can complete the attended transfer in a number of ways
depending on the current bridged states of the channels involved.
The hiding of masquerades is done in some bridging-related functions, such as the manager
Bridge action and the Bridge dialplan application. In addition, call pickup was edited
to "move" a channel rather than masquerade it.

This may alleviate some of the CDR woes with originated channels, as CDRs
do like to know when a channel was originated. Eventually this will get
converted to be a channel flag, so its location is still good to know
post the great CDR shakeup of 2013.

In r388005, macros were introduced to consistently define message
types. This added an assert if a message type was used either before
it was initialized or after it had been cleaned up. It turns out that
this assertion fires during shutdown.
This actually exposed a hidden shutdown ordering problem. Since
unsubscribing is asynchronous, it's possible that the message types
used by the subscription could be freed before the final message of
the subscription was processed.
This patch adds stasis_subscription_join(), which blocks until the
last message has been processed by the subscription. Since joining was
most commonly done right after an unsubscribe, a
stasis_unsubscribe_and_join() convenience function was also added.
Similar functions were also added to the stasis_caching_topic and
stasis_message_router, since they wrap subscriptions and have similar
problems.
Other code in trunk was refactored to join() where appropriate, or at
least verify that the subscription was complete before being
destroyed.

In the move from Asterisk's event system to Stasis, this makes
distributed device state aggregation always-on, removes unnecessary
task processors where possible, and collapses aggregate and
non-aggregate states into a single cache for ease of retrieval. This
also removes an intermediary step in device state aggregation.

* A new Stasis payload has been defined for multi-channel messages. This
payload can store multiple ast_channel_snapshot objects along with a single
JSON blob. The payload object itself is opaque; the snapshots are stored
in a container keyed by roles. APIs have been provided to query for and
retrieve the snapshots from the payload object.
* The Dial AMI events have been refactored onto Stasis. This includes dial
messages in app_dial, as well as the core dialing framework. The AMI events
have been modified to send out a DialBegin/DialEnd events, as opposed to
the subevent type that was previously used.
* Stasis messages, types, and other objects related to channels have been
placed in their own file, stasis_channels. Unit tests for some of these
objects/messages have also been written.

This patch started out simply as fixing the bouncing tests introduced
in r382685, but required some other changes to give it a decent
implementation.
To fix the bouncing tests, the UserEvent and Newexten AMI events
needed to be refactored to dispatch via Stasis. Dispatching directly
to AMI resulted in those events sometimes getting ahead of the
associated Newchannel events, which would understandably confuse anyone.
I found that instead of creating a zillion different message types and
structures associated with them, it would be preferable to define a
message type that has a channel snapshot and a blob of structured data
with a small bit of additional information. The JSON object model
provides a very nice way of representing structured data, so I went
with that.
* Move JSON support from res_json.c to main/json.c
* Made libjansson-dev a required dependency
* Added an ast_channel_blob message type, which has a channel
snapshot and JSON blob of data.
* Changed UserEvent and Newexten events so that they are dispatched
via ast_channel_blob messages on the channel's topic.
* Got rid of the ast_channel_varset message; used ast_channel_blob
instead.
* Extracted the manager functions converting Stasis channel events to
AMI events into manager_channel.c.

For the initial use of this bus, I took some work kmoore did creating
channel snapshots. So rather than create AMI events directly in the
channel code, this patch generates Stasis events, which manager.c uses
to then publish the AMI event.
This message bus provides a generic publish/subscribe mechanism within
Asterisk. This message bus is:
- Loosely coupled; new message types can be added in seperate modules.
- Easy to use; publishing and subscribing are straightforward
operations.
In addition to basic publish/subscribe, the patch also provides
mechanisms for message forwarding, and for message caching.

The core module related to coloring terminal output was old and needed
some love. The main thing here was an attempt to get rid of the
obscene number of stack-local buffers that were allocated for no other
reason than to colorize some output. Instead, this uses a simple trick
to allocate several buffers within threadlocal storage, then
automatically rotates between them, so that you can make multiple calls
to the colorization routine within one function and not need to
allocate multiple buffers.

Taking advantage of the sorted order of the registered functions container
requires that they are actually inserted in the expected sort order.
* Insert the registered functions into the container in case sensitive
position. As a result, only the complete_functions() routine needs to
search the entire container because it does a case insensitive search for
convenience.

The '-' char is supposed to be ignored by the dialplan extension matching.
Unfortunately, it's treatment is not handled consistently throughout the
extension matching code.
* Made the old exten matching code consistently ignore '-' chars.
* Made the old exten matching code consistently handle case in the
matching.
* Made ignore empty character sets.
* Fixed ast_extension_cmp() to return -1, 0, or 1 as documented. The only
user of it in pbx_lua.c was testing for -1. It was originally returning
the strcmp() value for less than which is not usually going to be -1.
* Fix character set sorting if the sets have the same number of characters
and start with the same character. Character set [0-9] now sorts before
[02-9a] as originally intended.
* Updated some extension label and priority already in use warnings to
also indicate if the extension is aliased.

Fix misuses of timeouts throughout the code.
Prior to this change, a common method for determining if a timeout
was reached was to call a function such as ast_waitfor_n() and inspect
the out parameter that told how many milliseconds were left, then use
that as the input to ast_waitfor_n() on the next go-around.
The problem with this is that in some cases, submillisecond timeouts
can occur, resulting in the out parameter not decreasing any. When this
happens thousands of times, the result is that the timeout takes much
longer than intended to be reached. As an example, I had a situation where
a 3 second timeout took multiple days to finally end since most wakeups
from ast_waitfor_n() were under a millisecond.
This patch seeks to fix this pattern throughout the code. Now we log the
time when an operation began and find the difference in wall clock time
between now and when the event started. This means that sub-millisecond timeouts
now cannot play havoc when trying to determine if something has timed out.
Part of this fix also includes changing the function ast_waitfor() so that it
is possible for it to return less than zero when a negative timeout is given
to it. This makes it actually possible to detect errors in ast_waitfor() when
there is no timeout.

Remove the "RTP Read too short" warning for RTP keepalives. Remove the
the warning about the application delimiter switch from pipe to comma.
(You should've done this by now.) Make cdr_odbc report more when an
insert fails. Make chan_sip warn less when the peer wants SRTP (and we
don't) or sends a zero port to disable a media type.

This patch extends the extension state callbacks so that monitoring channels
(as chan_sip) get more information of the devices which are responsible for
an extension state change. The additional information is needed by chan_sip
to present names/numbers of the caller and callee in an early-state SIP
notification. Users of extenstion state callback not interested in the
additional information are not affected by the changes.
Motivation: to present the involved party's name/number in an early-state
nofification (used by the notified device as a pickup offer) one after another
so that a user can see which call he will pick up in an undirected pickup.
Such a pickup offer to a user shall indicate the same call (number/name-A calls
number/name-B) as the call which would be picked up when an undirected pickup
is executed.
Users interested in additional state info must use the new functions
ast_extension_state_add_extended() resp.
ast_extension_state_add_destroy_extended() to register an extended state
callback. When the callback is registered this way, an extra member
device_state_info of struct ast_state_cb_info is passed to the callback in
addition to the aggregated extension state. This container holds an object for
every device of the monitored extension hint consisting of the device name, the
device state and a channel reference to the channel which (presumably) caused
the device state.
The information is used by chan_sip for early-state notifications. When the
state of a device changes and the new state contains AST_EVENT_RINGING, an
early-state notification is sent to the subscribed devices with the
caller/callee names/numbers of the oldest ringing channel of the monitored
extension. The notified user may then invoke a direct pickup, which will pickup
exactly this channel.
Users of the old non-extended callbacks will only be called when the aggregated
state did change (same behavior as before). Users of the extended callback will
also be called when the state is unchanged but does contain AST_EVENT_RINGING.
That could be the case if two channels are ringing at one device and one of
them hangs up, so the aggregated state does not change. This way the monitoring
channel can create a new early-state notification with the now ringing
party-ids.

This is based on the work done by Olle Johansson on review board.
The idea is that the channel specified in an AMI originate or call
file is typically not connected to the outgoing extension until the
channel has been answered. With this change, an EarlyMedia header can
be specified for AMI originates and an early_media option can
be specified in call files. With this option set, once early media is
received on a channel, it will be connected with the outgoing extension.

2) The patch was originally credited as the one uploaded by Guenther
Kelleter (license 6372) via issue AST-921, but the patch committed
was not the patch referenced on the issue.
3) Guenther Kelleter's patch was actually correct. It moved the
ast_free above the presencechange_cleanup label. I am not
committing his change as it is not technically necesary--calling
ast_free(NULL) is perfectly safe and I worry that moving the
ast_free outside of the label could lead to future bugs if
someone ever adds another failure conditional and expects
'goto presencechange_cleanup;' to clean up after everything.

This patch adds some basic documentation for a number of modules. This
includes core source files in Asterisk (those in main), as well as
chan_agent, chan_dahdi, chan_local, sig_analog, and sig_pri. The DTD
has also been updated to allow referencing of AMI commands.

Hangup handlers are an alternative to the h extension. They can be used
in addition to the h extension. The idea is to attach a Gosub routine to
a channel that will execute when the call hangs up. Whereas which h
extension gets executed depends on the location of dialplan execution when
the call hangs up, hangup handlers are attached to the call channel. You
can attach multiple handlers that will execute in the order of most
recently added first.

Add support-level indications to many more source files.
Since we now have tools that scan through the source tree looking for files
with specific support levels, we need to ensure that every file that is
a component of a 'core' or 'extended' module (or the main Asterisk binary)
is explicitly marked with its support level. This patch adds support-level
indications to many more source files in tree, but avoids adding them to
third-party libraries that are included in the tree and to source files
that don't end up involved in Asterisk itself.

The Hangup application used to just return -1 to cause normal dialplan
execution to hangup a channel. For the non-normal execution routines like
predial and connected-line interception routines, the hangup request would
exit the routine early but otherwise be ignored.
* Made the Hangup application not allow setting a cause code of zero. A
zero cause code is not defined.

Presence support has been added. This is accomplished by
allowing for presence hints in addition to device state
hints. A dialplan function called PRESENCE_STATE has been
added to allow for setting and reading presence. Presence
can be transmitted to Digium phones using custom XML
elements in a PIDF presence document.
Voicemail has new APIs that allow for moving, removing,
forwarding, and playing messages. Messages have had a new
unique message ID added to them so that the APIs will work
reliably. The state of a voicemail mailbox can be obtained
using an API that allows one to get a snapshot of the mailbox.
A voicemail Dialplan App called VoiceMailPlayMsg has been
added to be able to play back a specific message.
Configuration hooks have been added. Configuration hooks
allow for a piece of code to be executed when a specific
configuration file is loaded by a specific module. This is
useful for modules that are dependent on the configuration
of other modules.
chan_sip now has a public method that allows for a custom
SIP INFO request to be sent mid-dialog. Digium phones use
this in order to display progress bars when files are played.
Messaging support has been expanded a bit. The main
visible difference is the addition of an AMI action
MessageSend.
Finally, a ParkingLots manager action has been added in order
to get a list of parking lots.

This patch addresses a number of memory leaks in a variety of modules that were
found by a static analysis tool. A brief summary of the changes:
* app_minivm: free ast_str objects on off nominal paths
* app_page: free the ast_dial object if the requested channel technology
cannot be appended to the dialing structure
* app_queue: if a penalty rule failed to match any existing rule list
names, the created rule would not be inserted and its memory
would be leaked

method will now not allocate the ast_sockaddr struct if it
will return failure.
* pbx: cleanup hash table traversals in off nominal paths; free
ignore pattern buffer if it already exists for the specified
context
* xmldoc: cleanup various nodes when we no longer need them
* main/editline: various cleanup of pointers not being freed before being
assigned to other memory, cleanup along off nominal paths
* menuselect/mxml: cleanup of value buffer for an attribute when that attribute
did not specify a value
* res_calendar*: responses are allocated via the various *_request method
returns and should not be allocated in the various
write_event methods; ensure attendee buffer is freed if no
data exists in the parsed node; ensure that calendar objects
are de-ref'd appropriately
* res_jabber: free buffer in off nominal path
* res_musiconhold: close the DIR* object in off nominal paths
* res_rtp_asterisk: if we run out of ports, close the rtp socket object and free
the rtp object
* res_srtp: if we fail to create the session in libsrtp, destroy the
temporary ast_srtp object

This patch improves the handling of call id logging significantly with regard
to transfers and adding APIs to better handle specific aspects of logging.
Also, changes have been made to chan_sip in order to better handle the creation
of callids and to enable the monitor thread to bind itself to a particular
call id when a dialog is determined to be related to a callid. It then unbinds
itself before returning to normal monitoring.

The VoiceMail d([c]) option is documented to accept digits for a new extension
in context <c>, if played during the greeting. This option works fine if the
extension being redirected to has an extension with the same initial digit in
the channel's current context. If that digit did not happen to exist in some
extension, a dialplan match would fail and the user would not be redirected.
This patch fixes it such that if the <c> option is used, the extensions are
matched in that context as opposed to the caller's original context.

Fix AMI module reload deadlock regression from ASTERISK-18479 when it
tried to fix the race between calling an AMI action callback and
unregistering that action. Refixes ASTERISK-13784 broken by
ASTERISK-17785 change.
Locking the ao2 object guaranteed that there were no active callbacks that
mattered when ast_manager_unregister() was called. Unfortunately, this
causes the deadlock situation. The patch stops locking the ao2 object to
allow multiple threads to invoke the callback re-entrantly. There is no
way to guarantee a module unload will not crash because of an active
callback. The code attempts to minimize the chance with the registered
flag and the maximum 5 second delay before ast_manager_unregister()
returns.
The trunk version of the patch changes the API to fix the race condition
correctly to prevent the module code from unloading from memory while an
action callback is active.
* Don't hold the lock while calling the AMI action callback.

* Added 'b' and 'B' options to Dial. These options will allow you to run
last-minute dialplan on the caller and callee channels while the Dial
application is executing, but before the call is started. For example you
can use the 'b' option to run dialplan on the callee channel to get the name
of the newly created channel right away.

This change permits each verbose destination (consoles, logger) to have its
own concept of what the verbosity level is. The big feature here is that
the logger will now be able to capture a particular verbosity level without
condemning each console to need to suffer that level of verbosity.
Additionally, a stray 'core set verbose' will no longer change what will go
to the log.

There are many benefits to making the ast_channel an opaque handle, from
increasing maintainability to presenting ways to kill masquerades. This patch
kicks things off by taking things a field at a time, renaming the field to
'__do_not_use_${fieldname}' and then writing setters/getters and converting the
existing code to using them. When all fields are done, we can move ast_channel
to a C file from channel.h and lop off the '__do_not_use_'.
This patch sets up main/channel_interal_api.c to be the only file that actually
accesses the ast_channel's fields directly. The intent would be for any API
functions in channel.c to use the accessor functions. No more monkeying around
with channel internals. We should use our own APIs.
The interesting changes in this patch are the addition of
channel_internal_api.c, the moving of the AST_DATA stuff from channel.c to
channel_internal_api.c (note: the AST_DATA stuff will have to be reworked to
use accessor functions when ast_channel is really opaque), and some re-working
of the way channel iterators/callbacks are handled so as to avoid creating fake
ast_channels on the stack to pass in matching data by directly accessing fields
(since "name" is a stringfield and the fake channel doesn't init the
stringfields, you can't use the ast_channel_name_set() function). I went with
ast_channel_name(chan) for a getter, and ast_channel_name_set(chan, name) for a
setter.
The majority of the grunt-work for this change was done by writing a semantic
patch using Coccinelle ( http://coccinelle.lip6.fr/ ).

Chan_sip gives a dialog reference to the extension state callback and
assumes that when ast_extension_state_del() returns, the callback cannot
happen anymore. Chan_sip then reduces the dialog reference count
associated with the callback. Recent changes (ASTERISK-17760) have
resulted in the potential for the callback to happen after
ast_extension_state_del() has returned. For chan_sip, this could be very
bad because the dialog pointer could have already been destroyed.
* Added ast_extension_state_add_destroy() so chan_sip can account for the
sip_pvt reference given to the extension state callback when the extension
state callback is deleted.
* Fix pbx.c awkward statecbs handling in ast_extension_state_add_destroy()
and handle_statechange() now that the struct ast_state_cb has a destructor
to call.
* Ensure that ast_extension_state_add_destroy() will never return -1 or 0
for a successful registration.
* Fixed pbx.c statecbs_cmp() to compare the correct information. The
passed in value to compare is a change_cb function pointer not an object
pointer.
* Make pbx.c ast_merge_contexts_and_delete() not perform callbacks with
AST_EXTENSION_REMOVED with locks held. Chan_sip is notorious for
deadlocking when those locks are held during the callback.
* Removed unused lock declaration for the pbx.c store_hints list.

If waitExten specifies a music class to use with its music on hold option, it will use
CHANNEL(musicclass) instead if that channel variable has been set on the initiating
channel. This documents that behavior in the waitExten app so that this can be known
without checking the documentation of the code in function local_ast_moh_start.

Prior to this patch, if labels with the same name were used for different priorities in
the same extension, the new label would be accepted, but it would be unusable since
attempts to reach that label would just go to the first one. Now pbx.c detects this,
generates a warning in logs, and culls the label before adding it to the dialplan.

When an extension is removed from a context, its entry in the pattern match
tree is not deleted. Instead, the extension is marked as deleted. When an
extension is removed and re-added, if that extension is also a prefix of
another extension, several log messages would report an error and did not
check whether or not the extension was deleted before accessing the memory.
Additionally, if the extension was already in the tree but previously
deleted, and the pattern was at the end of a match, the findonly flag was
not honored and the extension would be erroneously undeleted.
Additionaly, it was discovered that an IAX2 peer could be unregistered
via the CLI, while at the same time it could be scheduled for unregistration
by Asterisk. The unregistration method now checks to see if the peer
was already unregistered before continuing with an unregistration.

Load the proper XML documentation when multiple modules document the same application.
This patch adds an optional "module" attribute to the XML documentation spec
that allows the documentation processor to match apps with identical names from
different modules to their documentation. This patch also fixes a number of
bugs with the documentation processor and should make it a little more
efficient. Support for multiple languages has also been properly implemented.

Fix deadlock when using dummy channels.
Dummy channels created by ast_dummy_channel_alloc() should be destoyed by
ast_channel_unref(). Using ast_channel_release() needlessly grabs the
channel container lock and can cause a deadlock as a result.
* Analyzed use of ast_dummy_channel_alloc() and made use
ast_channel_unref() when done with the dummy channel. (Primary reason for
the reported deadlock.)
* Made app_dial.c:dial_exec_full() not call ast_call() holding any channel
locks. Chan_local could not perform deadlock avoidance correctly.
(Potential deadlock exposed by this issue. Secondary reason for the
reported deadlock since the held lock was part of the deadlock chain.)
* Fixed some uses of ast_dummy_channel_alloc() not checking the returned
channel pointer for failure.
* Fixed some potential chan=NULL pointer usage in func_odbc.c. Protected
by testing the bogus_chan value.
* Fixed needlessly clearing a 1024 char auto array when setting the first
char to zero is enough in manager.c:action_getvar().

This is a bug - or architecture mistake - that has been in Asterisk for a
very long time. It was exposed by the AMI originate action and possibly
some other applications. Most channel drivers checks if an extension
exists BEFORE starting a pbx on an inbound call, so most calls will
not depend on this issue.

Make CANMATCH with the new pattern match engine behave more like the old one
When checking an extension for E_CANMATCH using the new extension matching
algorithm, an exact match was not returned as a possible match resulting in the
queue failing to allow a caller to exit on DTMF. This removes the requirement
that an extension be longer than acquired digits for an E_CANMATCH operation
to succeed.

Updated SIP 484 handling; added Incomplete control frame
When a SIP phone uses the dial application and receives a 484 Address
Incomplete response, if overlapped dialing is enabled for SIP, then
the 484 Address Incomplete is forwarded back to the SIP phone and the
HANGUPCAUSE channel variable is set to 28. Previously, the Incomplete
application dialplan logic was automatically triggered; now, explicit
dialplan usage of the application is required.
Additionally, this patch adds a new AST_CONTOL_FRAME type called
AST_CONTROL_INCOMPLETE. If a channel driver receives this control frame,
it is an indication that the dialplan expects more digits back from the
device. If the device supports overlap dialing it should attempt to
notify the device that the dialplan is waiting for more digits; otherwise,
it can handle the frame in a manner appropriate to the channel driver.

Deadlocks dealing with dialplan hints during reload.
There are two remaining different deadlocks reported dealing with dialplan
hints.
The deadlock in ASTERISK-17666 is caused by invalid locking order in
ast_remove_hint(). The hints container must be locked before the hint
object.
The deadlock in ASTERISK-17760 is caused by a catch-22 situation in
handle_statechange(). The deadlock is caused by not having the conlock
before calling the watcher callbacks. Unfortunately, having that lock
causes a different deadlock as reported in ASTERISK-16961.
* Fixed ast_remove_hint() locking order.
* Made handle_statechange() no longer call the watcher callbacks holding
any locks that matter.
* Made hint ao2 destructor do the watcher callbacks for extension
deactivation to guarantee that they get called.
* Fixed hint reference leak in ast_add_hint() if the callback container
constructor failed.
* Fixed hint reference leak in complete_core_show_hint() for every hint it
found for CLI tab completion.
* Adjusted locking in ast_merge_contexts_and_delete() for safety.
* Added context_merge_lock to prevent ast_merge_contexts_and_delete() and
handle_statechange() from interfering with each other.
* Fixed ast_change_hint() not taking into account that the extension is
used for the hash key.

Reset our ast_str before passing it on to dialplan function backends.
It is possible for a dialplan backend to not modify the given buffer or ast_str
and still return success. This causes any previous value stored in the buffer
to be used as if the new function call provided it. Some functions also append
to the given buffer assuming it is empty.
The test_substitution unit test has also been modified to detect this problem.

Fixes locking inversion issue in ast_async_goto()
During this function we can not hold the "chan" lock while
doing the masquerade, the explicit goto on the tmp chan, or
the channel alloc. Instead we need to get the channel lock,
store off information about the channel that we need, and
then let the channel lock go for the remainder of the function.

The 'e' special extension fails to trigger in at least two cases.
The 'e' extension is a fall back for the 'i', 't', or 'T' extensions if
any of them do not exist. Many of the places the 'e' extension was
supposed to be invoked fail because the priority was set wrong. There
were two places where the 'e' extension was not even checked for fall
back.
* Made invoke the 'e' extension similarly to the previous 'i', 't', or 'T'
extension check and added the 'e' extension as a fall back to the two
missing locations.
* Prioritized and optimized some hangup tests associated with the 'e'
extension.

Fix channel redirect out of MeetMe() and other issues with channel softhangup.
Mantis issue #18585 reports that a channel redirect out of MeetMe() stopped
working properly. This issue includes a patch that resolves the issue by
removing a call to ast_check_hangup() from app_meetme.c. I left that in my
patch, as it doesn't need to be there. However, the rest of the patch fixes
this problem with or without the change to app_meetme.
The key difference between what happens before and after this patch is the
effect of the END_OF_Q control frame. After END_OF_Q is hit in ast_read(),
ast_read() will return NULL. With the ast_check_hangup() removed, app_meetme
sees this which causes it to exit as intended. Checking ast_check_hangup()
caused app_meetme to exit earlier in the process, and the target of the
redirect saw the condition where ast_read() returned NULL.
Removing ast_check_hangup() works around the issue in app_meetme, but doesn't
solve the issue if another application did the same thing. There are also
other edge cases where if an application finishes at the same time that a
redirect happens, the target of the redirect will think that the channel hung
up. So, I made some changes in pbx.c to resolve it at a deeper level. There
are already places that unset the SOFTHANGUP_ASYNCGOTO flag in an attempt to
abort the hangup process. My patch extends this to remove the END_OF_Q frame
from the channel's read queue, making the "abort hangup" more complete. This
same technique was used in every place where a softhangup flag was cleared.

by splitting up devices from hints into an own ao2_container the callback to
get these devices for statechange handling is faster.
with this changes the length of a device used in a hint isnt longer restricted
to 80 characters.
Tests showed that calling handle_statechange is 40 times faster if no hints
are used and 25 times faster if there are any hints.

The channel redirect function (CLI or AMI) hangs up the call instead of redirecting the call.
To recreate the problem:
1) Party A calls Party B
2) Invoke CLI "channel redirect" command to redirect channel call leg
associated with A.
3) All associated channels are hung up.
Note that if the CLI command were done on the channel call leg associated
with B it works.
This regression was a result of the fix for issue #16946
(https://reviewboard.asterisk.org/r/740/).
The regression affects all features that use an async goto to execute the
dialplan because of an external event: Channel redirect, AMI redirect, SIP
REFER, and FAX detection.
The struct ast_channel._softhangup code is a mess. The variable is used
for several purposes that do not necessarily result in the call being hung
up. I have added doxygen comments to describe how the various _softhangup
bits are used. I have corrected all the places where the variable was
tested in a non-bit oriented manner.
The primary fix is the new AST_CONTROL_END_OF_Q frame. It acts as a weak
hangup request so the soft hangup requests that do not normally result in
a hangup do not hangup.

Change new pattern matcher to regard dashes the same as the old pattern matcher -- as visual candy to be ignored.
Also change the AEL parser to not generate dashes within extensions, as those
dashes would be ignored. Update the AEL tests to match this behavior.
