Ensure the return value of ast_bridge_call is passed back up as the application return value. This is needed for transfers to function so the PBX core knows to continue execution.

Luigi Rizzo 0595b5e2aa include "logger.h" and errno.h from asterisk.h - usage shows that they 
were included almost everywhere.
Remove some of the instances.

Luigi Rizzo fdb7f7ba3d Start untangling header inclusion in a way that does not affect 
build times - tested, there is no measureable difference before and
after this commit.

In this change:

use asterisk/compat.h to include a small set of system headers:
inttypes.h, unistd.h, stddef.h, stddint.h, sys/types.h, stdarg.h,
stdlib.h, alloca.h, stdio.h

Where available, the inclusion is conditional on HAVE_FOO_H as determined
by autoconf.

Normally, source files should not include any of the above system headers,
and instead use either "asterisk.h" or "asterisk/compat.h" which does it
better. 

For the time being I have left alone second-level directories
(main/db1-ast, etc.).

Kevin P. Fleming edc78d6023 improve linked-list macros in two ways: 
- the *_CURRENT macros no longer need the list head pointer argument
  - add AST_LIST_MOVE_CURRENT to encapsulate the remove/add operation when moving entries between lists

Basically, this changes ...

to ...

Joshua Colp 96a646734f It is no longer required for each module that deals with a channel to call ast_module_user_hangup_all in it's unload function. The loader will automatically perform this action for it.

Steve Murphy 6dcb17baaf This update fulfils the request of bug 7109, which claimed the language arg to ast_stream_and_wait() was redundant. Almost all calls just used chan->language, and seeing how chan is the first argument, this certainly seems redundant. A change of language could just as easily be done by simply changing the channel language before calling.

Matt O'Gorman 05a695af72 everything that loads a config that needs a config file to run 
now reports AST_MODULE_LOAD_DECLINE when loading if config file
is not there, also fixed an error in res_config_pgsql where it 
had a non static function when it should.
